I thought that was Freeman's fault also to be honest. Very long toss to go underhand. Ayala was expecting an overhand strong throw and then he was going to find the bag. He got an underhand throw and it didn't leave him much time to find it. Just bad play all around.

Generally you see 2 PHs used when the pitcher's spot is due up, the first PH comes in, the opposing team changes pitchers to get the platoon advantage, and the first team changes hitters again to regain the platoon advantage.
