Somehow MLB has created a situation where a pitcher can throw at a hitter, have the hitter's team effectively barred from revenge, and have the hitter's manager ejected.
This is what happens when baseball players aren't allowed to police the game themselves.
And then later in the game hit someone else, accident or not, and not get punished. So our best player gets hit in the ribs on the first pitch of the game, our manager ejected, everyone warned, the same pitcher hits another one of our players and incurs zero punishment. Yeah, that makes total sense. If these are the new rules I hope we come out tomorrow and absolutely smoke their lead off hitter in the ribs. Take our warning and then prevent them from hitting us without ejections.

With the way MLB does it, the aggressor is hugely favored. The Marlins were able to drill Acuna knowing that if Keuchel even grazed a Marlin, it would be seen as retaliation and Keuchel would be tossed. This means Keuchel has to be extra careful about going inside (a benefit for the Marlins).
If Keuchel does accidentally hit someone, he gets ejected and the Braves have to nuke their pen for the rest of the series (another possible good result for the Marlins).
And since the Marlins were the initial aggressor, they had not motive for revenge. Hitting a batter later in the game was clearly an accident and so Hernandez wasn't tossed.
MLB's direction on how to handle these situations allows a team to throw at a batter and then enjoy all kind of protections while not really suffering any drawback. It makes zero sense.
If you're not going to toss Hernandez then you don't need to warn the benches. Either let the players police it or toss the pitcher whenever you have to warn the benches. A team having to go to their pen after one pitch would be motivation for managers to keep a leash on their pitchers.
