The point about externalities is risk taking that is not internalized. It is the difference between risking say skin cancer which is not contagious and a contagious disease.

Regular people couldn't get this drug simply by asking for it.

This is the anti-intellectual we are relying on to lead us through this pandemic, and his complete and utter incompetence in all matters of life is why we still can't get folks tested at the required rates.
