Does it make a difference if the first TJS is at a really young age? I‘d think most pitchers getting their second one are already well in their thirties.

Could be. There isn't nearly the data on a 2nd TJS compared to the 1st one. But coming back 100% from a second one is just super rare where for the 1st one it's almost expected at this point.
