That's a fallacy of composition. Public health policy, including large amounts of nagging, has and does move the needle. It has done so with respect to smoking. With respect to vaccines.
 
Ok. It doesn’t follow that therefore we then must have this position called the surgeon general. I’d be happy to see it go. And if it turns out that one less government bureaucrat in a sea of thousands means we’re not nagging enough at the margin, then maybe we reassess.
 
I'm being pedantic. Of course, we should try to be smarter in our use of tax dollars. But sometimes you need to allocate the money and resources. It will never be used perfectly. Those opposed to government policy of any kind will always find plenty of examples of fraud and waste.
 
Interestingly, it was under Reagan that we had a very successful intervention that had to be coordinated globally to protect the environment and public health. This was the treaty that led to the banning and phasing out of certain compounds that were depleting the ozone layer. Once upon a time conservatives were sensible people.
 
What is under funded ?
A tragic example of a public good that that was underfunded would be the alarms and sensors that warn people of flooding. Texas and other red states are often quite cheap about funding public goods.

On a larger scale the failure of certain states to take up the Medicaid expansion made possible by the ACA. The results have been very favorable in the states that have adopted it.

As the article stated, the position is redundant. It was created for a particular purpose that no longer exists. Thanks to mission creep, it morphed into an overly politicized bully pulpit. Between the director of HHS and heads of various health agencies, there are plenty of places to distribute the “duties” of the Surgeon General.

However, you provided further description rather than suggest a detailed how you would fix it
Eliminating the figurehead will not solve the problems you explain.
Nor will , I think you will agree throwing out the baby with the bath water
Which I take it is the sturg view
