You can pinch-hit as often as you like until you run out of players, at any time. You can only change pitchers once per batter (IOW a pitcher has to face at least one batter before being removed from the game, barring injury).

Generally you see 2 PHs used when the pitcher's spot is due up, the first PH comes in, the opposing team changes pitchers to get the platoon advantage, and the first team changes hitters again to regain the platoon advantage.
